  • Life Care Planning

A Life Care Plan (LCP) is a dynamic document based on published standards of practice, comprehensive assessment, data analysis, and research, which provides an organized, concise plan for current and future needs with associated costs for individuals who have experienced catastrophic injury or have chronic health care needs.

Our Life Care Planning specialist reviews medical records and supporting documentation, conducts a comprehensive interview, consults with appropriate therapeutic team members, prepares preliminary life care plan opinions, researches associated costs and sources, and finalizes the report by confirming projects. Included in the final report is an itemized description of the services, the duration of the services, how frequently the services will be required, the purpose of the service, and the associated costs per unit and per year.
